I declare that I have ensured the following requirements have been met in accordance with my scope of works:
- » I have received and read a copy of the WHS Management Plan for the project and discussed relevant sections applicable to my work with the principal contractor, where required.
- » I have discussed relevant sections of the Plan, including the site rules, with my workers and subcontractors.
- » Safe Work Method Statements (SWMSs) have been prepared for all “high risk construction work” being undertaken by myself and/or my workers and subcontractors, and for any other work as requested by the principal contractor.
- » All workers and subcontractors engaged by my business have been:
- » Copies of all current documents have been forwarded to the principal contractor:
- » Any tools, plant and equipment to be used on site are in good, safe working order.
- » All mobile plant to be used by my business on site is properly maintained and complies with all relevant WHS laws and Department of Transport and Main Roads requirements.
- » My WorkCover policy covers all workers engaged by my business.
I also agree to ensure myself and my subcontractors and workers abide by all provisions of the principal contractor’s WHS management plan and any relevant industrial, health and safety, electrical safety and workers’ compensation laws applicable to my work at the work site.
